چکیده

Purpose: The overexpression of P-glycoprotein (Pgp) is thought to be an important mechanism of pharmacoresistance in epilepsy. Recently, [C]phenytoin has been evaluated preclinically as a tracer for Pgp. The aim of the present study was to assess the optimal plasma kinetic model for quantification of [C]phenytoin studies in humans. Procedures: Dynamic [C]phenytoin PET scans of 6 healthy volunteers with arterial sampling were acquired twice on the same day and analyzed using singleand 2-tissue-compartment models with and without a blood volume parameter. Global and regional test– retest (TRT) variability was determined for both plasma to tissue rate constant (K1) and volume of distribution (VT). Results: According to the Akaike information criterion, the reversible single-tissuecompartment model with blood volume parameter was the preferred plasma input model. Mean TRT variability ranged from 1.5% to 16.9% for K1 and from 0.5% to 5.8% for VT. Larger volumes of interest showed better repeatabilities than smaller regions. A 45-min scan provided essentially the same K1 and VT values as a 60-min scan. Conclusion: A reversible single-tissue-compartment model with blood volume seems to be a good candidate model for quantification of dynamic [C]phenytoin studies. Scan duration may be reduced to 45 min without notable loss of accuracy and precision of both K1 and VT, although this still needs to be confirmed under pathologic conditions.

A quantitative phenytoin GC-MS method and its validation for samples from human ex situ brain microdialysis, blood and saliva using solid-phase extraction.

This study describes the development and validation of a gas chromatography-mass spectrometry (GC-MS) method to identify and quantitate phenytoin in brain microdialysate, saliva and blood from human samples. A solid-phase extraction (SPE) was performed with a nonpolar C8-SCX column. The eluate was evaporated with nitrogen (50°C) and derivatized with trimethylsulfonium hydroxide before GC-MS ana...

Positive propylene glycol result in a patient with ethylene glycol poisoning.

A 61-year-old man presented with an increased anion-gap metabolic acidosis, an increased serum osmolal gap, and a negative result in an alcohol screen. Gas chromatography revealed an increased ethylene glycol (EG) concentration (22 mg/dL) and a propylene glycol (PG) result that was below the lower limit of quantification ( 5 mg/dL) (Fig. 1A). The patient was treated with hemodialysis, followed ...

Influence of chronic phenytoin administration on the pharmacokinetics and pharmacodynamics of vecuronium.

BACKGROUND The duration of action of vecuronium is reduced in patients receiving phenytoin. In this study, the authors examined, simultaneously, the influence of phenytoin on both the pharmacokinetics and the pharmacodynamics of vecuronium. METHODS This study was approved by the institutional review board of the University of California, San Francisco, and patients gave written informed conse...
